Job Responsibilities:
· Assist in planning and executing wellness, recreational and social programmes and activities for the seniors.
· Facilitate essential support services like transportation, healthcare referrals and social support for seniors.
· Organise activities that promote social interaction, mental stimulation and physical health.
· Assist in managing, supervising and supporting volunteers while fostering a positive and collaborative environment.
· Administer assessment tool to ensure buddying & befriending criteria are met.
· Assist in conducting outreach to raise awareness and promote the centre’s services within the community.
· Prepare reports on programme participation and other relevant data and maintain accurate records.
· Provide general administrative support, including scheduling, event preparation and communications while adhering to PDPA guidelines.

Job Requirements:
· Minimum of a Diploma in a related discipline or its equivalent.
· At least 2 years of working experience in programme coordination.
· Good communication & interpersonal skills.
· Preferably with an interest and passion for working with the elderly.
· Related qualification in working with the elderly or in the social service sector, would be an advantage.

Job Responsibilities:
· Conduct case management, work and counselling support and group work for residents under your care.
· Conceive intervention plan and execution which can effect positive change in residents.
· Work closely with MSF and other agencies on the overall rehabilitation of residents.
· Prepare in-depth care plan and other mandatory monthly reports as required by MSF and other agencies.
· Plans, executes, and evaluates programmes and services with the aim to improve the quality of life of residents.
· Attend case conferences, presents cases for discussion on formal platforms such as departmental or inter-agencies meetings for the purpose of sharing and learning problem solving skills relating to social work.
· Attends multi-disciplinary team meetings with medical staff, nurses, and other allied healthcare professionals to discuss, propose and coordinate care plans for residents.
· Development and enhancement of work process and administrative matters.
· Any other duties assigned by Reporting Officer.

Job Requirements:
· Must possess degree or graduate diploma in Social Work.
· Must possess compassion, resourcefulness and be willing to work diligently to help in the rehabilitation of male destitute persons.
· Registered with SASW Accreditation Board or SAC or SPS preferred with at least 2 years of experience. Fresh graduates are welcomed.
· Good report writing and interpersonal skills.
· A good team player.
· Willing and comfortable to work in an all-male institution.

Roles & Responsibilities
• Assist the Nursing staff in providing the clients with basic hygiene and personal care on a daily basis
• Maintain up-to-date documentation of client’s conditions and interventions and support development and review of care plans
• Support the Programme team to facilitate both on and off-site activities with the clients
• Perform tasks and daily activities with the client according to the care plans
• Maintains cleanliness and tidiness of the client’s environment
• Taking and monitoring of vital signed
• Serving food and drinks during mealtimes
• Interacting, talking and exercising with the elderly and assist in toileting

Requirements:
• Minimum GCE O Level / HMI Certificate in Healthcare Support (Nursing) or equivalent or
• Nitec in Community Care & Social services would be advantageous
• Relevant working experience in a healthcare setting preferred
• Must have the passion to serve the elderly and comfortable to handle dementia clients
