Roles & Responsibilities:

Client Management for DDCC & MDC

  • To manage and follow up all-new e-referrals from AIC.
  • To do initial assessment for ADLs, such as evaluating a client’s environmental psycho-social and health needs.
  • Monitor and verify services each client receives and evaluate the effectiveness of each intervention.
  • To manage cases and provide family counselling services.
  • Provide referral information to clients when there is a need for other services.
  • Provide support to the clients by getting information through Social Workers from hospitals, polyclinics, family service centres, and doctors.
  • Co-ordinate continuity of care following discharges from hospitals with the hospital case coordinator, Doctor-In-Charge, or pharmacy.

Administrative duties

  • Administrative duties include completing new client intake, gathering supporting documents and managing case files.
  • Document progress for each client as monitored by the program coordinator.
  • Conduct interviews to determine client eligibility.
  • Discharge planning.
  • Liaise with Caregiver/Family Member.
  • Supervise all claims e.g., Subvention & SMF.
  • Supervise application of Senior Mobility Funds.
  • Seek alternative funds to support the clients’ financial assistance.

Requirements:

    • Bachelor’s Degree in social work
    • Minimum 2 years of relevant experience in Social Service/Healthcare sector
    • Team player with good interpersonal, organisational and communication skills
    • Familiarity with the system and schemes such as SMF and Medifund will be an advantage.

    Roles & Responsibilities:

    • Supports and assists the Staff Nurse/Enrolled Nurse in the delivery of care to residents in the nursing home in accordance to established policies and procedures, standards or nursing care and practice.
    • Assists in the physical admission, transfer, and discharge of residents to and from the ward.
    • Assist in preparing resident for physical examination and/or other minor procedures.
    • Performing and maintenance of resident’s hygiene needs – diaper changing, bathing, hair care, oral care, and skin care.

    Requirements:

    • Minimum certification in Nursing from a recognised institution/Secondary School or equivalent.
    • Preferably 1-year relevant experience as a Nursing Aide.
