Social Worker/Social Work Associate/Community Care Executive (Senior Care Centre)
Roles & Responsibilities:
- Manage and follow up new e-referrals from AIC.
- To do client assessment for ADLs, functional, environmental psycho-social and health needs.
- Monitor and client services and evaluate the effectiveness of each intervention.
- To manage cases and provide family counselling services.
- Provide referral information to clients when there is a need for other services.
- Provide support to the clients through Social Workers from hospitals, polyclinics, family service centres, and doctors.
- Coordinate with the hospital case coordinator or Doctor-In-Charge or pharmacy to ensure essential care is provided for clients.
- Complete new client intake and managing case files.
- Document client progress.
- Conduct interviews to determine client eligibility.
- Perform discharge planning.
- Liaise with Caregiver or Family Members of clients.
- Supervise all claims e.g., Subvention & SMF.
- Supervise application of Senior Mobility Funds.
- Support the clients’ financial requirements by recommending funds or schemes they can tap on.
Job Requirements:
- Bachelor’s Degree in Social Work.
- Minimum 2 years of relevant experience in Social Service/Healthcare sector.
- Team player with good interpersonal, organisational and communication skills.
- Familiarity with the healthcare system and health schemes such as SMF and Medifund will be an advantage.
